To Families and Guardians of People with Developmental Disabilities
Greetings!
On behalf of the Persons with Developmental Disabilities (PDD) South Board, I invite you to participate in our 2008 Family & Guardian Satisfaction Survey.
We conduct this survey every two years to assess your level of satisfaction with the PDD-funded services available to your family member or the person whom you represent. Your responses are very important and help us to better understand your needs and expectations.
In addition to the paper survey, new this year for your convenience, you can complete the survey online using the link provided in your survey package. If you complete the survey online, please do not complete the paper survey included in the package. You should only complete 1 version of the survey.
All survey responses are anonymous and you will not be personally identified in any way. If you complete the paper survey, please send your completed surveys to the Vocational and Rehabilitation Research Institute (VRRI) using the stamped and self-addressed envelope enclosed in your survey package. If you complete the survey online, you should not send in the paper copy of the survey as your responses will be electronically recorded. The VRRI will analyze the survey results independently and prepare a report.
